About

Trinity Hospital’s Swing Bed program is an interdisciplinary approach to caring for patients who no longer require acute hospital care, but are not ready to safely or successfully discharge home.


As a Critical Access Hospital, we have applied for and received approval from CMS to provide post-acute care rehabilitation services by “swinging” our acute beds to rehabilitation beds. You can receive these services without having to transfer to Redding or Sacramento or even further.


If you spend your acute stay at a facility outside of Weaverville, our Intake Coordinators/Discharge Planners can work with your current care team to bring you back home to Trinity County to receive your post-acute care rehabilitation services locally.

As set by CMS, to qualify for our Swing Bed program and receive up to 100 days of rehabilitation services, you will need an acute inpatient stay at Trinity Hospital or another hospital of at least three consecutive midnights within the past 30 days.
