About

Our federally designated Rural Health Clinics (RHCs) provide medically essential primary care services such as: urgent care, patient physicals, treatment of minor illnesses, preventive care, and chronic disease management. In addition, we help facilitate continuity of care locally along with referring to specialty care if needed. Our staff of Physicians, Family Nurse Practitioners (FNP), Physician Assistants (PA-C) coupled with our strong group of Certified Medical Assistants, Referral Coordinators, and Administrative/Clerical Staff are here to help increase access to healthcare to our local communities.


Both clinics offer special programs: Medication Assisted Treatment, Substance Use Disorder, Hepatitis C treatment and laboratory services.

Opioid Treatment Program

In 2016, an estimated 7% of people ages 12 years and older (824 people) misused opioids in Trinity County and 1.3% of people (149 people) had an opioid use disorder (OUD), defined as opioid abuse or dependence. As a result of these statistics, Mountain Communities saw a need in the community for a Medication Assisted Treatment Program (MAT).

Medicated Assisted Treatment (MAT) is the use of FDA-approved medications, in combination with counseling and behavioral therapies, to provide a “whole-patient” approach to the treatment of substance use disorders.

Our program commences in early October 2019. Our current providers that are certified to prescribe Suboxone, a prescription medication used to treat opioid addiction, are Heidi Johnson, NP and Matt Butler, PA-C.

The clinics are accepting new patients into the Medicated Assisted Treatment Program.

Substance Use Disorder (SUD) can be a devastating disease but seeking the proper treatment can help you recover and find a better way to live.

Substance Use Disorder is a medical condition that is defined by the inability to control the use of a particular substance (or substances) despite harmful consequences.

Substances are drugs that have addition potential. They can be prescribed drugs or non-medical drugs.

Our facility offers:

·         Medicated Treatment for Alcohol and Opioid management

·         Onsite Tele-Med counselors and psychiatry

·         Referral Department to assist with specialty referrals

It is important to seek help as soon as possible if you think you or a loved one is developing SUD.

Patient Financial Services

Thank you for choosing Mountain Communities Healthcare District (MCHD) for your healthcare needs.

The Patient Financial Services Department is here Monday through Friday from 8:00am to 5:00pm to assist you with any billing questions. The Patient Financial Services Department has a dedicated staff that works diligently to ensure that your claims are submitted to your insurance company promptly. Once it has been determined that the balance is the patient’s responsibility, a statement is mailed.

A financial counselor is available to assist you with your bill, set up payment plans, or see if you or your family member qualifies for any state/federal programs or hospital discount programs.

You may receive a separate bill for:

·         Emergency Room Physician

·         Radiologist

·         Surgeon

·         Pathologist

·         Outside Reference Labs – if the lab test cannot be performed at MCHD.

No one will be denied access to services due to inability to pay. MCHD has a financial assistance program (discounted/sliding fee) based on family size and income.
