Part time, benefited position available on our weekend day shift. Must have current CA RN license in good standing, BLS and ACLS certifications are required. Ability to work within a team environment, demonstrated leadership skills, critical thinking skills and the ability to multi-task in a fast paced environment are required. Wage range is $50-$70/hour DOE, plus benefits. 

We have one full time, benefited position available on our NOC shift (7.5% shift differential) and a casual, non-benefited position available in our Skilled Nursing Facility, on both day shift and on NOC shift (7.5% shift differential). Some of the position responsibilities are as follows: Prepares and administers medications and treatments as prescribed by physician/physician assistant. Notes time, amount and reactions; May take and record blood pressure, pulse, temperature and respiration; and performs various therapeutic procedures such as sterile and non-sterile dressing, collections of urine, stool, sputum and wound drainage for specimens. We are seeking a candidate with a current CA LVN license in good standing as well as a current BLS. Our ideal candidate will have at least one year of experience as an LVN in a Skilled Nursing Facility; however we are willing to consider a new grad for these positions. Starting wage is $35/hour, plus benefits.

We have a full time, benefited position available.  Some of the position responsibilities are as follows: performs administrative functions such as evaluating epidemiology activities, resident care from an infection control standpoint as well as performing studies as recommended/needed; serves as a resource person for orientation, patient care, staff education, employee health, and development of infection control standards; coordinates infection control surveillance activities; Investigates infection clusters and unusual pathogens; reports required communicable disease to Public Health Dept. Minimum qualifications are as follows: must have primary professional training as a licensed nurse, medical technologist, microbiologist, epidemiologist, public health professional, or other healthcare related field. Candidates must be qualified by education, training, clinical or healthcare experience, or certification, and must have completed specialized training in infection prevention and control. Starting wage is $35 – $50/hour, plus benefits depending upon licensure and experience.

We have a full time, benefited position available. Some of the position responsibilities are as follows: Provides resource information and support for all nursing staff within the parameters of the DSD’s nursing State licensure; Provides at least 20 hours per week of staff development resource to the Skilled Nursing Facility CNAs; Planning, implementing and evaluating at least 16 hours of new-hire NA/CNA orientation; Providing “CNA Quarterly” classes: Providing 24 hours of required CNA in-service training for all CNAs employed by the facility; Active Trainer in any Nursing Aide Certification Program, under the general supervision of the CNO (Title 42, CFR 483.152); completes quarterly and annual assessments with reporting to CMS in timely fashion; Monitors completion of MDSs by other disciplines within timeframes prescribed by regulatory guidelines; Ensures accurate, timely completion of the MDS sheet for assigned residents. Qualifications of this position are as follows: Current, clear California Vocational Nurse License or a Registered Nurse License; Current BLS certification; Current California DSD Certification or obtain within 6 months of hire; Obtain BLS Instructor Certification within 6 months of hire (preferred); Two years of nursing experience (RN or LVN); One year verifiable experience as a licensed nurse providing care and services to chronically ill or elderly patients in an acute care hospital, SNF, ICF, home care, hospice care, or other LTC facility. Completion of a course in teaching adults OR 1 year of verifiable experience in teaching adults OR 1 year of verifiable experience supervising nurse aides; DSDs must be approved by CDPH in order to teach; Knowledge of MDS and Care Plan process. Basic knowledge of and RUGs and CMS. Starting wage for an LVN is $37/hour and RN is $52/hour DOE, plus benefits. 

We have a full time, benefited, management position available. Some of the position responsibilities are as follows: Plans, develops, implements and evaluates patient/resident care, personnel and fiscal management systems in conjunction with Administration, Patient Services and other department directors; Coordinates and participates, with various employee groups and teams, continuous quality assurance and CQI and compliance activities; Understands Trinity Hospital policies and procedures for hiring, evaluation and corrective action. Facilitates, as needed, proper professional interaction; Maintains policy and procedure manual following Title 22 and OBRA(CMS) guidelines for this area. Instructs staff on content and proper use; Ensures that management and supervisory coverage is scheduled for all absences from facility; Coordinates and attends SNF quarterly meetings as scheduled; Provides educational programs and activities for employees as necessary and/or assigned; Plans, develops, implements, monitors and evaluates CQI indicators for the SNF. Facilitates involvement of employees and Medical Staff. Minimum qualifications for the position are as follows: Associate Degree of Diploma in Nursing required; BSN preferred; CPR certification required; Minimum of two years clinical experience in Skilled Nursing Facilities required; Prior management experience preferred; Demonstrated ability to apply theories of personnel and fiscal management systems in the Skilled Nursing Environment; Current California Registered Nurse License required. Wage range is $140k-$160k/annually DOE, plus benefits.
